Pentosan Polysulfate (Elmiron)
FDA-approved
01 · Overview
A semi-synthetic sulfated polysaccharide (not a peptide) derived from beechwood hemicellulose, with structural similarity to glycosaminoglycans and heparin. Pentosan polysulfate is thought to work by supplementing the defective glycosaminoglycan layer of the bladder lining in interstitial cystitis, reducing the permeability of the urothelium to irritants.
It is the only FDA-approved oral drug for interstitial cystitis (Elmiron, approved 1996) (PMID 34000253).
02 · Research summary & key findings
A meta-analysis of four placebo-controlled trials found pentosan better than placebo for pain, urgency and frequency, but not nocturia (PMID 9218016); a later 368-patient trial found no difference from placebo (PMID 25245489). Long-term use is linked to a dose-dependent, vision-threatening pigmentary maculopathy (Hanif et al., Ophthalmology, 2019), and retinal screening is recommended (PMID 34000253).
