Carperitide (h-ANP)
No lawful US pathway
01 · Overview
Carperitide is a recombinant form of human atrial natriuretic peptide (alpha-human A-type natriuretic peptide), given intravenously (PMID 25999241). It activates guanylyl cyclase A receptors, raising intracellular cyclic GMP to promote vasodilation, sodium excretion, and diuresis, thereby reducing cardiac loading conditions.
It is marketed in Japan for acute heart failure but has never been approved by the FDA or in Europe.
02 · Research summary & key findings
Carperitide has been used in a large share of acute heart failure admissions in Japan, yet its clinical effectiveness is not well documented (PMID 25999241). A propensity-score-matched Japanese analysis linked carperitide to higher in-hospital mortality (PMID 25999241), and a 2025 meta-analysis of randomized and propensity-matched studies found higher in-hospital mortality overall but no significant difference when only randomized trials were pooled, and no benefit for long-term mortality or rehospitalization (PMID 41310450). The randomized LASCAR-AHF trial of low-dose carperitide found no reduction in death or heart failure hospitalization at 2 years, though it was stopped early and underpowered (PMID 39656827).
03 · Regulatory status
Carperitide (h-ANP) is in the investigational stage. While preclinical and early-phase data exist, it has not received regulatory approval for clinical use in the United States.

Carperitide (h-ANP) is not FDA-approved and does not currently have a lawful prescription or compounding pathway in the United States, so we do not sell it and will not book a consultation for it. That can change: in 2026 FDA began reviewing several peptides for compounding.
