Octreotide (Sandostatin)
Overview
A synthetic octapeptide analog of somatostatin with a substantially longer half-life (90 minutes IV, 6 hours subcutaneous vs. 2 minutes for native somatostatin).
Mechanism of Action
Octreotide binds somatostatin receptors (primarily SSTR2 and SSTR5) to inhibit the secretion of GH, glucagon, insulin, gastrin, secretin, VIP, and other GI hormones. It reduces splanchnic blood flow, GI motility, and exocrine pancreatic secretion.
Research Summary & Key Findings
FDA-approved for acromegaly, carcinoid syndrome (diarrhea/flushing), and VIPoma-associated watery diarrhea. Long-acting release (LAR) formulation provides once-monthly dosing. PROMID trial showed antiproliferative effect in midgut neuroendocrine tumors with 66% risk reduction for disease progression (Rinke et al., J Clin Oncol, 2009). Used off-label for variceal bleeding, pancreatic fistula, and refractory diarrhea.
