Iseganan (IB-367)
Overview
Iseganan is a 17-amino acid synthetic analog of protegrin, a naturally occurring antimicrobial peptide found in porcine leukocytes. It disrupts microbial cell membranes via a beta-sheet structure stabilized by disulfide bonds, conferring broad-spectrum activity against bacteria, fungi, and some viruses.
Mechanism of Action
Iseganan was developed primarily as an oral rinse to prevent ventilator-associated pneumonia and oral mucositis in immunocompromised patients. Its rapid microbicidal action and low systemic absorption profile supported its formulation for topical oral use.
Research Summary & Key Findings
Phase III trials in ventilator-associated pneumonia prevention did not meet primary endpoints for reducing infection rates, leading to discontinuation of development for that indication. Studies in chemotherapy-induced oral mucositis similarly failed to demonstrate significant clinical benefit. Development was halted in the early 2000s following these trial results.
Clinical Status
Iseganan (IB-367) is in the investigational stage. While preclinical and early-phase data exist, it has not received regulatory approval for clinical use in the United States.
