01 · Overview

Eltrombopag is an orally active, small-molecule, non-peptide thrombopoietin receptor agonist. Although it is not itself a peptide, it activates thrombopoietin receptor signalling to stimulate megakaryocyte proliferation and platelet production.

Its oral availability addresses a limitation of the first-generation recombinant thrombopoietin proteins, whose development was abandoned because of neutralizing antibodies (PMID 20620436). Promacta was approved under NDA 022291 and remains listed in the FDA drug approval database, where generic eltrombopag olamine products are also now listed.

02 · Research summary & key findings

Eltrombopag received FDA approval in November 2008 according to the FDA drug approval database, and the current label covers persistent or chronic immune thrombocytopenia with insufficient response to corticosteroids, immunoglobulins or splenectomy, thrombocytopenia in chronic hepatitis C to allow interferon-based therapy, and severe aplastic anemia. The RAISE trial, a 6-month randomized placebo-controlled phase 3 study published in The Lancet in 2011, not NEJM in 2010, found that 79 percent of 135 patients given eltrombopag responded at least once, compared with 28 percent of 62 patients on placebo, with fewer patients needing rescue treatment and fewer serious bleeding events (PMID 20739054). Thromboembolic events and liver enzyme increases occurred more often with eltrombopag in that trial (PMID 20739054).
