01 · Overview
A long-acting GLP-1 receptor agonist consisting of a GLP-1 analog fused to a modified human IgG4 Fc fragment, a design that retained full receptor activity while greatly extending half-life compared with native GLP-1 (PMID 20503261). In patients, it reaches peak concentration in about 48 hours and has a terminal elimination half-life of about 5 days, supporting once-weekly dosing (PMID 26507721).
Dulaglutide activates GLP-1 receptors to enhance glucose-dependent insulin secretion and suppress glucagon.
02 · Research summary & key findings
In the AWARD phase 3 program, dulaglutide was superior to exenatide, insulin glargine, metformin and sitagliptin for HbA1c reduction and non-inferior to liraglutide (PMID 27102969). The REWIND trial (2019) showed a 12% reduction in major adverse cardiovascular events in type 2 diabetes with and without prior cardiovascular disease (PMID 31189511). FDA-approved for type 2 diabetes (2014) and cardiovascular risk reduction.
